State Life Insurance

Life insurance is available to employees who are eligible for SEGIP benefits. Coverage is provided through the State of Illinois, and is administered by MetLife. Go to MyBenefits to make changes to your life insurance enrollment.

Eligibility

You are eligible for basic life insurance coverage of 1x your annual salary if you are eligible for SEGIP benefits.

You may also purchase optional additional life insurance coverage for yourself, as well as for your spouse/civil union partner and children. Children age 25 and under are eligible, except individuals in the Other category. Children in the Disabled category are eligible for life coverage as long as they continue to meet eligibility requirements.

Cost

There is no premium cost for basic life insurance. Up to $50,000 of basic coverage is tax exempt, and amounts in excess of $50,000 are taxed as imputed income.

If you elect optional employee coverage, premium rates are determined based on your age, annual salary, and the amount of coverage you elect. Spouse/civil union partner and child coverage are a flat rate for a fixed level of coverage. Spouse/civil union partner coverage is reduced upon attaining age 60. Your premium costs for optional coverage are paid through payroll deduction.

Basic Employee Term Life Coverage

If you are eligible for SEGIP, then you are automatically enrolled in basic term life insurance coverage of 1x your annual salary through MetLife.

Optional Employee Term Life Coverage

You may elect to add optional life insurance coverage in specified amounts of 1x up to 8x your annual salary, to a maximum of $3,000,000 when combined with your basic life insurance. You may increase your optional life coverage at any time, but the following conditions apply:

  • If you add optional life coverage within 30 calendar days of your SEGIP eligibility date:
    • You may elect up to 4x your annual salary with guaranteed enrollment and coverage will be effective on your eligibility date.
    • If you choose to apply for coverage of 5x or more, you must provide a Statement of Health and coverage will be effective on the date the Statement of Health is approved by the insurer.
  • If you elect optional life coverage at any other time (including during the annual Benefit Choice period) you must provide a Statement of Health. Coverage will be effective on the date the Statement of Health is approved by the insurer. Access the MetLife Statement of Health form (also known as Evidence of Insurability). 

Optional Spouse and Child Coverage

You may add spouse/civil union partner and/or child coverage at any time, but the following conditions apply:

  • If you add spouse/civil union partner coverage within 30 calendar days of your SEGIP eligibility date, enrollment is guaranteed without a Statement of Health. Coverage will be effective on your eligibility date.
  • If you add child coverage at any time, enrollment is guaranteed without a Statement of Health.
  • If you add spouse/civil union partner at any other time (including during the annual Benefit Choice period) you must provide a Statement of Health. Coverage will be effective on the date the Statement of Health is approved by the insurer. Access the MetLife Statement of Health form (also known as Evidence of Insurability). 

Beneficiaries

It is important that you name a beneficiary to receive your life insurance benefits in the event of your death. Without a beneficiary designation, in the event of your death, benefits can be delayed and may exclude those you wished to receive benefits. Having a beneficiary designation on file and reviewing your beneficiary designation periodically will ensure that your insurance benefits will be distributed more quickly, and to the individuals you specified.

To review, add, or update your beneficiary online:

  • Go to https://www.metlife.com/stateofillinois/ and select the View/Change Beneficiary button
  • Select Register to perform the one-time registration process or Login if you are already registered
    • For your protection, you may be asked to verify your identity before you can access your account
  • Select My Accounts or View My Accounts, then select Add or Update Beneficiary
  • Select Group Life Insurance
  • On the Life Coverage Summary screen, select Beneficiaries or Add/Update Life Beneficiaries   
    • Select Next after adding or updating your beneficiaries
    • Review info for accuracy, then select Next
    • Check the “I acknowledge . . “ box and in the Password box, enter your Date of Birth (DOB) as YYYYMMDD, then Submit
    • Print your Beneficiary Designation form for your records

You may also access the MetLife Beneficiary Designation form. Mail completed forms to the address on the form.
