Form SSA-1945

Employees who who start a SURS-eligible job at the University of Illinois should review and submit the Social Security Administration Form SSA-1945.

Form SSA-1945 contains information about the the possible effects of the Windfall Elimination Provision and the Government Pension Offset Provision on an employee's potential future Social Security Benefits.
